Job summary

Douglas Manor, a 90‑bed skilled nursing facility, seeks a full‑time Social Worker to support resident well‑being and navigate complex care transitions. You will coordinate psychosocial services, facilitate communication between residents, families, and the care team, and connect individuals with community resources.

We value a compassionate professional with a BSW (MSW preferred), CT licensure eligibility, and 1–2 years in long‑term care.

Qualifications

• BSW required; MSW preferred and LCSW/CMSW eligibility considered.
• Connecticut LMSW eligible or licensed preferred.
• 1–2 years of social work in long‑term care or healthcare settings.

Responsibilities

• Assess residents' psychosocial needs and develop care plans.
• Provide counseling to residents and families for adjustment and grief.
• Advocate for residents' rights and coordinate with care teams.
• Facilitate communication among residents, families, and staff.
• Coordinate discharge planning and community resource access.
• Document assessments, plans, and communications accurately.
• Support staff education on psychosocial needs and crisis intervention.
