Job summary

Douglas Manor Nursing and Rehabilitation Center seeks a dedicated full-time Social Worker to support resident well-being, coordinate psychosocial services, and advocate for resident rights within a compassionate, person-centered care environment.

You will collaborate with residents, families, and the care team to facilitate seamless transitions and connect individuals with community resources, while maintaining thorough documentation and compliance with regulations.

Qualifications

• BSW required; MSW preferred.
• Active CT LMSW licensure or eligibility; LCSW preferred.
• 1–2 years of professional social work experience, preferably with older adults or in long-term care or healthcare settings.
• Experience in psychosocial assessment, care coordination, discharge planning, and case management.
• Knowledge of state and federal long-term care regulations and resident rights.
• Strong interpersonal and communication skills; ability to work collaboratively with residents, families, and multidisciplinary care teams.
• Demonstrated ability to provide crisis intervention, counseling, and supportive services.
• Excellent organizational and time-management skills, with the ability to prioritize multiple responsibilities and manage documentation.

Responsibilities

• Conduct comprehensive psychosocial assessments of residents and develop individual care plans.
• Provide counseling and support to residents and families for adjustment, grief, stress, and related issues.
• Advocate for residents' rights, preferences, and needs to ensure quality of life.
• Facilitate communication among residents, families, and the care team.
• Coordinate discharge planning and connect residents with community resources.
• Assist residents in accessing community services and benefits as appropriate.
• Participate in care conferences to align psychosocial goals with overall care planning.
• Maintain accurate documentation of assessments, interventions, and communications.
• Serve as a resource to staff on psychosocial needs and staff education as needed.
• Provide crisis intervention and support in emergencies.
