Job summary

The Advocate Healthcare Center of East Boston is hiring a Social Services Designee – Full-Time to support residents and families during their stay. You will conduct psychosocial assessments, assist with discharge planning, and coordinate community resources to ensure compassionate, coordinated care.

Collaborate with nursing, admissions, and administration to help residents feel safe, respected, and supported. The role emphasizes resident-centered care, advocacy, and strong teamwork.

Qualifications

• Experience in Social Services in healthcare or LTC setting.
• Strong communication, empathy and problem-solving skills.
• Bachelor’s in Social Work or related field preferred.
• LCSW or SW degree required.
• Minimum 1 year of SNF experience.

Responsibilities

• Conduct psychosocial assessments for new admissions
• Provide support, counseling, and advocacy to residents and families
• Assist with discharge planning and community resource coordination
• Maintain documentation in compliance with state and federal regulations
• Participate in care plan meetings with an interdisciplinary team
• Address resident concerns and promote a positive, respectful environment
• Support transitions of care and family communications
