Job summary

Sellersburg Healthcare Center, a member of the CommuniCare Family, seeks a Social Services Designee to support discharge planning, advocate for residents, and collaborate with care teams to enhance residents’ psychosocial well-being.

You will develop rapport with residents and staff, ensure privacy, and address changes in condition with nursing leadership while promoting dignity and quality of life.

Qualifications

• Social Services Designee experience in long-term care is required.
• Experience working with residents, families, and care teams to support quality of life.

Responsibilities

• Work with Social services, Therapy and clinical teams to establish discharge planning for residents.
• Build relationships with team and residents to support residents' psychosocial well-being.
• Report grievances and maintain documentation for the Director of Social Services.
• Advocate for residents and coordinate with direct care staff to meet needs.
• Protect confidentiality of resident/family information at all times.
