Qualifications

Responsibilities

• Assist residents and their families with social, emotional, and family issues.
• Provide timely and accurate documentation to comply with Facility, State, and Federal policies.
• Participate in resident care planning and discharge planning.
• Serve as a general information source for residents and their families.
