Job summary

Signature HealthCARE, LLC is seeking a Social Services Director for Rockcastle Health & Rehabilitation Center. You will identify residents’ social, emotional, and psychological needs and assist in discharge planning to support smooth transitions.

The role requires a Bachelor’s or Master’s in Social Work or Psychology and 3+ years in social services, preferably in healthcare, with a strong focus on confidentiality and professionalism. Part-time days during standard business hours are offered.

Qualifications

• Bachelor’s or Master’s Degree in Social Work or Psychology.
• At least 3 years of experience in social services, preferably healthcare.
• High level of professionalism and ability to maintain confidentiality.

Responsibilities

• Identify and address residents’ social, emotional, and psychological needs throughout their stay.
• Assist in planning for discharge and coordinating discharge planning.
• Provide information and support to residents and families; participate in care planning.
