Job summary

Rockcastle Health & Rehabilitation Center, part of Signature HealthCARE, is seeking a Social Services Director to address residents’ social, emotional, and psychological needs and to support discharge planning and ongoing development during stays.

You will coordinate resident care planning, documentation, and communications with families, while upholding confidentiality and professional standards in a skilled nursing environment.

Qualifications

• Bachelor’s or Master’s Degree in Social Work or Psychology.
• 3+ years of experience in social services, preferably in healthcare.
• High level of professionalism and ability to maintain confidentiality.

Responsibilities

• Assist residents and families with social, emotional, and family issues.
• Provide timely and accurate completion of documentation to comply with Facility, State, and Federal policies.
• Participate in resident care planning.
• Participate in discharge planning and prepare resident for the transition.
• Serve as a general information resource for residents and families.
