Job summary

Signature HealthCARE is seeking a Social Services Director for a resident-centered facility in Kentucky. You will identify and support residents’ social, emotional, and psychological needs and assist in discharge planning to ensure smooth transitions.

Requirements include a Bachelor’s or Master’s in Social Work or Psychology and 3+ years of healthcare social services experience, with a strong emphasis on confidentiality and professionalism.

Qualifications

• Bachelor’s or Master’s Degree in Social Work or Psychology.
• 3+ years of experience in social services, preferably in healthcare.
• Ability to maintain confidentiality and professionalism.

Responsibilities

• Assist residents and their families with social, emotional, and family issues.
• Provide timely and accurate completion of documentation needed to comply with Facility, State, and Federal policies.
• Participate in resident care planning.
• Participate in the development of Discharge planning and prepare resident for the transition.
• Serve as a general source of information for residents and their families.
