Job summary

Signature HealthCARE is seeking a Social Services Director to identify and address residents' social, emotional, and psychological needs during their stay, and assist in discharge planning for smooth transitions.

You will lead care planning, coordinate with families, and ensure timely, compliant documentation while upholding confidentiality and professional standards relevant to a healthcare setting.

Qualifications

• Bachelor’s or Master’s Degree in Social Work or Psychology.
• 3+ Years Experience working in Social Services, directly with individuals, preferably in a healthcare setting.
• High level of professionalism and ability to maintain confidentiality.

Responsibilities

• Assist residents and their families with social, emotional, and family issues.
• Provide timely and accurate completion of documentation needed to comply with Facility, State, and Federal policies.
• Participate in resident care planning.
• Participate in the development of Discharge planning and prepare resident for the transition.
• Serve as a general source of information for residents and their families.
