Responsibilities

• Complete forms, reports and notes with informative descriptions.
• Coordinate social services programs with residents, families, staff and community agencies.
• Provide information on support agencies and financial assistance.
• Develop preliminary and comprehensive resident assessments and care plans.
• Encourage resident participation in care plans.
• Review and revise care plans quarterly.
• Maintain rapport with services for coordinated care.</br>Maintain knowledge of regulations and confidentiality.
• Ensure residents' rights and emotional and social needs are met.
• Report complaints to Social Services Director.
