Job summary

Desoto Health is seeking a Social Service professional to support residents in a nursing home setting. The role involves coordinating pre-admission activities, assisting residents and families with orientation, and serving as the discharge coordinator.

Job DetailsJob Location: ARCADIA, FL 34266The Social Service functions in behalf of the nursing home and it's residents by promoting the general well being of each resident through identification of their emotional and psychosocial needs. The SS works with staff in developing a means to meet and sustain these needs. The SS will also assist supportive others and community sources in servicing the nursing home and its residents

Qualifications

• Works directly with the other department supervisors and Social Work Consultant if required.
• Handles the inquiries received by the facility.
• Coordinates the pre-admission procedures, handouts, and required paperwork.
• Assists the resident, responsible party, and supportive others in the adjustment and orientation to the nursing home after admission.
• Assures timely progress notes on each resident in the facility.
• Participates in each resident's total plan of care staffing. Oversees that resident/families are invited and informed of staffing.
• Assures a Social History is completed within fifteen (15) days following an admission.
• Is the Discharge Coordinator for the facility.
• Is the Grievance intake person for the facility.
• Has knowledge of the community resources available and use them to benefit and improve the quality of care as applicable for each resident.
• Assures timely progress notes on each resident.
• Takes advantage of opportunities to inform and make community aware of the nursing home's need and value to the community. Examples: writing
• newspaper articles, talking with influential people in the community, facilitating workshops with the community about nursing homes.
• Has an open-door policy to all residents, family members, and staff to listen to their problems or needs; do effective counseling or referrals to meet these needs.
• Attends seminars, in-services, convention, and other educational opportunities to keep informed, improve personal knowledge and share experiences.
• Dresses in a professional manner, with cleanliness and tidiness as a priority.
• Assists other departments within the facility when requested, and when it will benefit the residents and/or help the facility operate smoothly.
• Is an advocate for the resident and his/her rights, helping the resident learn assertiveness in communicating their needs, wants, and problems to the appropriate departments and agencies.
• Meets with governmental services utilized by residents in the facility in preparation for discharge.
• Provides financial counseling and information to residents, inquiries, and responsible parties.
