Job summary

Rehabilitation and Healthcare Center of Tampa is seeking a Social Services Director to plan, implement, and evaluate the facility’s social service programs. You will work with administration, medical staff, and families to meet residents’ social, emotional, and financial needs.

The role focuses on coordinating discharge planning, guiding residents through Medicare/Medicaid options, and ensuring compliance with federal and state standards. Strong communication and analytical skills are essential.

Qualifications

• Must possess at least a high school diploma or equivalent; associate or bachelor’s degree preferred.
• Prior health-care experience preferred.
• Ability to read, analyze, and interpret general business materials.

Responsibilities

• Assist in planning, developing, organizing, implementing, evaluating, and directing social service programs.
• Meet with administration and staff to plan social services and policies.
• Provide information to residents/families about Medicare/Medicaid and financial assistance.
• Record progress notes and coordinate with other departments to maintain resident well-being.
• Assist in discharge planning and community resource referrals.
• Develop and maintain liaison with outside agencies to meet resident needs.
