Job summary

Ventura Services Florida is seeking a nursing professional to monitor chronic conditions, admit and orient new residents, and participate in discharge planning for a smooth transition to post-discharge care.

The role includes conducting comprehensive assessments, developing individualized care plans with the IDT, overseeing CNAs, and ensuring documentation in the EMR. Emphasis on compassionate, trauma-informed care and cultural sensitivity.

Qualifications

• Provide nursing care that is compassionate and sensitive to residents with cognitive decline, memory loss or history of trauma.
• Administer medications according to practitioner orders and report adverse effects or errors.
• Transcribe orders from providers as appropriate and document care in EMR.
• Collaborate with CNAs and coordinate with the interdisciplinary team to ensure resident needs are met.

Responsibilities

• Monitor the chronic health conditions of residents; be familiar with reportable changes and potential causes for concern.
• Assist in the admission process; provide resident orientation as appropriate.
• Assist with discharge planning focusing on discharge goals and transition to post-discharge care.
• Ensure that newly admitted residents have physician orders for immediate care.
• Perform initial, comprehensive, quarterly, change of condition and other resident assessments using MDS forms.
• Develop individualized, resident-centered care plans in coordination with the IDT.
• Provide nursing care that is compassionate and sensitive to residents with cognitive decline, memory loss or history of trauma.
• Provide nursing care that is appropriate and sensitive to the culture, language and background of the resident.
• Monitor resident weight and intake; notify practitioner of significant changes.
• Maintain documentation of nursing care in nurse's notes, flow sheets and EMR.
• Collaborate with hospice services to provide end-of-life palliative care.
• Promote falls prevention practices to maximize safety.
• Provide first aid as necessary and per facility protocols.
• Administer medications according to orders and transcribe/communicate accurately.
• Provide oversight of CNAs as directed.
• Assess and address resident pain in accordance with provider orders.
• Support personal care and ADLs; assist as needed.
• Assist bed-bound or chair-bound residents with repositioning.
• Monitor skin health; provide preventative skin care and wound treatments as ordered.
• Collect and submit specimens for laboratory analysis as ordered.
• Provide trauma-informed nursing care respecting individual experiences and triggers.
