Charlestown by Erickson Senior Living is seeking a resident-centered Physical Therapist with weekday availability to join our team. All residents live on our award-winning campus, providing an environment where you can be part of their journey throughout their continuum of care.

Our Physical Therapists support residents in all aspects of daily living, delivering person-centered care to meet their needs. Licensed PT with SNF experience encouraged to apply.

Qualificações

• Must possess a valid and current state Physical Therapy license.
• Inpatient and Outpatient experience highly preferred.
• Experience and the ability to complete documentation to meet reimbursement and regulatory requirements.
• Therapeutic experience in older adults across multiple diagnostic groups is preferred.
• Skilled Nursing Experience Required.

Responsabilidades

• Provide holistic, resident-centered care supporting physical, spiritual, emotional, and clinical needs.
• Implement residents’ care plans, focusing on strengths, preferences, and routines while offering comfort and belonging.
• Collaborate with the interdisciplinary clinical team to address residents’ needs throughout their care.
