EatonCreek is seeking a Staff Physical Therapist to evaluate residents, develop individualized treatment plans, and coordinate care with physicians and families. You will supervise PT assistants, provide in‑services, and maintain compliant documentation in a collaborative facility setting.

Strong communication skills and the ability to educate residents, families, and caregivers on mobility goals and home exercise programs are essential for success in this role.

Qualifications

• Licensed Physical Therapist with state license.
• Master's or Doctorate in Physical Therapy preferred.
• Proficient in Net Health and Point Click Care.
• CPR certification, if required by licensure.

Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop, implement, and update individualized treatment plans.
• Supervise Physical Therapy Assistants and aides.
• Communicate progress and discharge plans with physicians, families, and staff.
• Participate in resident care conferences and rehabilitation meetings.
• Provide in-services on training programs for staff as needed.
• Complete timely, accurate documentation per policies and payer requirements.
• Maintain a safe treatment environment and follow infection control protocols.
• Educate residents and families on home programs, safety, and fall prevention.
• Facilitate ordering of durable medical equipment to promote independence.
