Job summary

EATONCREEK is seeking a Staff Physical Therapist Assistant to support the Staff Physical Therapist in resident care at our Nashville facility. You will treat residents as directed, document accurately, maintain safety, and participate in care conferences.

The role requires an Associate Degree from an accredited PTA program, proficiency with Net Health and Point Click Care, strong communication, and the ability to educate residents and families. We offer an equal opportunity workplace.

Qualifications

• Associate degree from an accredited PTA program.
• Proficient in Net Health and Point Click Care.

Responsibilities

• Treat residents as directed by the Physical Therapist.
• Complete timely, accurate documentation per facility and payer requirements.
• Maintain a safe treatment environment following infection control and safety protocols.
• Participate in Resident Care conferences and Rehabilitation meetings.
• Educate residents, families, and caregivers on treatment goals and home programs.
• Communicate progress and discharge planning with the PT, nursing staff, and interdisciplinary team.
• Provide in-services on training programs for staff as needed.
• Recommend and facilitate durable medical equipment to aid mobility.
• Maintain confidentiality of residents' and company information.
• Support quality assurance and facility rehabilitation program development.
