Job summary

Ridgecrest Post Acute is seeking a dedicated Staff Physical Therapist to evaluate and treat residents, coordinate with families and physicians, and maintain thorough documentation in medical records. The role includes conducting in-services and ongoing training for facility staff.

You will evaluate residents, develop individualized treatment plans, supervise PT assistants, and communicate progress to the care team.

Qualifications

  • Bachelor's degree in Physical Therapy; Masters/Doctorate preferred.
  • Licensed as a Physical Therapist in the state.
  • CPR certification, if required by licensure.
  • Continuing education requirements per state practice.

Responsibilities

  • Evaluate residents to determine PT needs and rehab potential.
  • Develop and update individualized treatment plans to improve mobility and independence.
  • Supervise PT assistants and staff per state act.
  • Communicate with physicians, nurses, families about progress and goals.
  • Complete timely, compliant documentation per facility and payer requirements.
  • Provide in-services and training for facility staff as needed.
  • Maintain safe treatment environment and infection control protocols.
  • Assist with discharge planning and coordination with care teams.
