Job summary

Colonial Vista Community Healthcare, LLC is seeking caregivers to provide routine daily care to residents according to individualized plans. Qualifications

• Provide routine daily care and services according to resident care plans.
• Maintain accurate, timely notes and reports in medical records.
• Maintain confidentiality of resident information and follow privacy policies.

Responsibilities

• Assist with ALDS, meals, and daily activities.
• Greet residents and help with mobility and transfers.
• Record events and incidents clearly in notes and reports.
• Ensure rooms are prepared and safe for residents.
• Observe and report changes in resident condition to supervisors.
