Qualifications

• Experience in long-term care or residential care settings preferred.
• Ability to assist with lifting and transfers as directed.
• Strong observation and reporting of residents’ conditions.

Responsibilities

• Provides direct resident care promoting dignity, independence, and personal choice.
• Observes residents’ physical and mental condition and reports changes to health services leadership or Executive Director.
• Coordinates personal care with team members and responds to assistance requests promptly.
• Documents incidents per policy and completes required notifications.
• Assists residents to dining room and activities; serves meals and attends to mealtime needs.
• Performs first aid and initiates emergency procedures when required.
• Greets residents, visitors, and staff; answers phones when assigned.
• Supports fire safety and infection control practices and reports unsafe conditions.
• Assists with tours, move-ins/move-outs, and transfers.
• Assists with apartment housekeeping and resident laundry.
• Attends trainings, in-services, and staff meetings.
• Performs other duties as assigned.
