Job summary

Meadow View Assisted Living and Memory Care is seeking an overnight Caregiver to provide direct, hands-on care and supportive services. Under the supervision of the Wellness Director, you will promote resident dignity, privacy, choice, and independence while following individualized care plans and regulatory requirements.

Qualifications

• Assist residents with activities of daily living (ADLs) per individual care plans.
• Support resident mobility: transfers, ambulation, positioning, use of assistive devices.
• Assist residents to and from meals and provide mealtime support to promote nutrition, dignity, and safety.
• Measure and record vital signs, observe changes, and report promptly to wellness staff.
• Complete treatments or clinical tasks as ordered within scope of practice.
• Provide emotional support and maintain a calm, respectful presence.
• Document care and observations accurately in accordance with policy.
• Maintain open shift communication for continuity of care.
• Assist with light housekeeping and maintain a clean, safe environment as per care plan.
• Support personal laundry and return items as needed.
• Assist dining services with beverages, meal setup, and in-room dining support.
• Stock supplies and keep work areas clean per infection control standards.
• Follow safety, infection control, and resident rights protocols.
• Maintain confidentiality and comply with background checks and training requirements.
• Perform other duties as assigned.

Position Summary:

Under the supervision of the Wellness Director, the Caregiver is responsible for providing direct, hands‑on care and supportive services to residents at Meadow View Assisted Living and Memory Care. This role promotes resident dignity, privacy, choice, and independence while supporting the community’s philosophy of aging in place. Care is delivered in accordance with individual care plans, organizational policies, and all applicable regulatory requirements.

CAREGIVER ESSENTIAL FUNCTIONS

• Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, personal hygiene, toileting, and continence care, in accordance with individual care plans.
• Support resident mobility, including transfers, ambulation, positioning, and use of assistive devices as directed.
• Assist residents to and from meals and provide mealtime support as needed to promote nutrition, dignity, and safety.
• Measure and record vital signs, observe and report changes in resident condition, behavior, or functional status, and communicate concerns promptly to wellness leadership or nursing staff.
• Complete treatments or clinical support tasks as ordered and within the caregiver’s scope of practice.
• Provide emotional support and maintain a calm, respectful, and reassuring presence for residents at all times.
• Accurately and timely document care provided, observations, and services rendered in accordance with policy.
• Maintain open communication between shifts to ensure continuity of care and sharing of relevant resident information.
• Assist with light housekeeping duties in resident living areas, including bed making, linen changes, trash removal, and maintaining a clean, safe environment as outlined in the care plan.
• Support residents with personal laundry, including washing, drying, folding, and returning items as needed.
• Assist dining services with beverage service, meal setup, clearing tables, and in-room dining support as appropriate.
• Stock supplies, maintain clean and organized work areas, and clean shared equipment in accordance with infection control and safety standards.
• Follow all safety, emergency, infection control, and resident rights protocols.
• Maintain confidentiality and comply with all background check, training, and competency requirements as a condition of employment.
• Perform other duties as assigned consistent with the needs of the community and resident care.
