Job summary

Menlo Park Post Acute is seeking a dedicated CNA to provide routine daily nursing care to residents in our Portland facility. You will follow individualized care plans, identify residents before procedures, and ensure accurate documentation of observations and care activities.

The role emphasizes compassionate patient interaction, adherence to privacy policies, and timely reporting of changes in resident conditions to supervisors. CPR is preferred to enhance readiness for emergencies.

Qualifications

• Must possess, as a minimum, a 10th grade education.
• Must be a licensed Certified Nursing Assistant in accordance with state laws.
• CPR certification is preferred but not required to perform duties if not available.

Responsibilities

• Provide routine daily nursing care and services as per resident care plans.
• Identify residents using wristbands or photo IDs before delivering care.
• Record observations and care activities accurately on charts and flow sheets.
