Job summary

Sacramento Post Acute is seeking a dedicated Certified Nursing Assistant to provide routine daily nursing care per individual care plans. You will identify residents, assist with daily activities, and report changes to supervisors while maintaining confidentiality and following facility policies.

Requirements include state-licensed CNA and ability to perform nursing tasks, with CPR preferred. This role involves assisting residents with meals, mobility, and basic care in a clinical setting.

Qualifications

• Must be able to read, analyze, and interpret general business materials.
• Must be able to write reports and procedures.
• Must be able to present information to managers and employees.
• Licensed Certified Nursing Assistant in accordance with state laws; CPR preferred.

Responsibilities

• Provide routine daily nursing care per the resident care plan.
• Identify residents before administering treatments using wristbands or photo IDs.
• Report changes in residents' condition to the Nurse Supervisor/Charge Nurse promptly.
• Maintain confidentiality of all resident care information including HIPAA protections.
• Assist residents with daily activities and mobility as needed.
• Follow established policies and procedures and cooperate with staff to maintain quality care.
