Job summary

The Bishop McCarthy Center is seeking a Social Worker (LSW/CSW) to join a team-focused facility where residents and staff well-being come first. Competitive based on experience with a strong benefits package, overtime opportunities, and a referral program.

Responsibilities include assisting residents with emotional adjustment, conducting interviews, completing MDS/social care planning, and coordinating with interdisciplinary teams. CSW/LSW certification and 1–3 years' experience preferred.

Qualifications

• Bachelor's Degree in Social Work from an accredited college, required.
• CSW and/or LSW certification preferred or to be obtained.
• 1–3 years of professional social work experience or training, or equivalent education.

Responsibilities

• Assist residents with emotional adjustment to the facility.
• Interview new residents, record social history and assessments.
• Complete social services portion of the MDS and care planning.
• Refer to psychology services as needed.
• Attend interdisciplinary care plan meetings and participate in rounds.
• Write summaries and progress notes as directed.
• Assist with pre-discharge evaluations and discharge planning.
• Develop professional relationships with residents and families.
• Educate residents/representatives on Advanced Directives.
• Adhere to Social Services Regulations.
