Job summary

Qualifications

Responsibilities

• Complete Social Services portions of MDS, RAPs, Care Plans, and Progress Notes in compliance with regulations.
• Assist residents and families with social, emotional, and family issues.
• Identify social and emotional needs for care planning.
• Maintain progress notes and discharge planning records as required.
• Refer to support agencies via interdisciplinary team while documenting referrals.
• Coordinate monthly Family Council Meetings and family communications.
• Provide information to residents, families and the public as needed.
• Coordinate transportation, room changes, and care plan meeting invitations.
• Collaborate with Activity Coordinator to support residents without family support.
