Qualifications

• Bachelor's degree in Social Work or a human services field and one year of supervised work experience in healthcare.
• High School Diploma or equivalent.
• Neat appearance.
• Good physical and mental health.
• Good written and verbal communication skills.
• Good interpersonal skills.
• Good administrative and organizational abilities.
• Knowledge of social resources in the community.
• Degree in Psycho/Social Health area preferred.

Responsibilities

• Ensure residents' psychosocial and concrete needs are identified and met per federal, state and company guidelines.
• Orient residents to the center.
• Discharge planning.
• Implement and update Resident Care Plan and Social History.
• Maintain records of outside referrals.
• Assist in coordinating residents' financial affairs.
• Communicate needs and plan of care to residents, families, responsible parties and staff.
• Participate in meetings, in-services and training as required.
• Meet with consultants as required.
• Organize and facilitate Family Council as requested by members.
• Implement and maintain QMPI program for quality measurement and improvement.
• Assist with coordinating resident room moves.
