Job summary

Hampton Post Acute in Stockton, CA is seeking a Social Services Designee to join our team. The role supports residents and families with adjustment to placement and care plans, and assists admissions and discharge planning under the Social Services Director.

Must have a high school diploma or equivalent; experience with geriatric care is a plus. Competitive hourly pay and comprehensive benefits are offered.

Qualifications

• High school diploma or equivalent.
• AA or Bachelor's degree in a related field.
• Ability to organize, prioritize, and meet deadlines.
• Knowledge of nursing practices, procedures, terminology, laws, regulations, and guidelines pertaining to long-term care.
• Experience with geriatric population (Nice to Have).

Responsibilities

• Assist residents and families with adjustment to placement, illness, and plan of care.
• Participate in resident and family council meetings as requested.
• Support admissions and discharge planning services.
