Qualifications

Responsibilities

• Provide social services support to residents and their families throughout their stay.
• Assist residents with adjustment to the facility, emotional support, and psychosocial needs.
• Participate in resident assessments, care planning meetings, and interdisciplinary team conferences.
• Coordinate discharge planning and transitions to home or other care settings.
• Communicate with residents, families, healthcare providers, and community resources regarding care needs.
• Advocate for residents’ rights, dignity, privacy, and personal preferences.
• Maintain confidential documentation in accordance with policies and regulations.
• Support residents and families with concerns, grievances, and referrals to services.
