Hampton Post Acute in Stockton, CA is hiring a Social Services Designee to support residents and families with placement, treatment plans, and transitions. Under the Social Services Director, you help with discharge planning, council meetings, and ensuring person-centered care.

The role emphasizes confidentiality, respect, and adherence to federal and state regulations, with a focus on teamwork and compassionate service to elderly residents and their families.

Qualifications

• High school diploma or equivalent.
• Ability to work with physically and mentally ill, disabled, and emotionally upset elderly individuals.
• Ability to organize, prioritize, and meet deadlines.
• Ability to maintain confidentiality and be discreet.
• Knowledge of nursing practices, procedures, terminology, laws, regulations, and guidelines that pertain to long-term care.
• Experience with geriatric population.
• AA or Bachelor's degree in related field.
• Creative problem-solving skills.
• Sufficient computer skills to manage records and documentation.

Responsibilities

• Under the direction of the Social Services Director, assist each resident and family in adjusting to placement, illness, and the plan of care.
• Help residents and families with wishes regarding treatment and transitions to the facility.
• Participate in resident and family council meetings as requested.
• Support Admissions and assist with discharge planning services.
