Job summary

The Goodman Group, LLC seeks a Social Services Assistant to support planning, organizing and directing the Social Services Department in our Oregon community. You will ensure residents receive emotional and social support per regulations and policies, under the direction of the Director of Social Services.

Responsibilities include completing initial assessments, developing care plans, coordinating with care teams, assisting with discharge planning and connecting residents to Medicare/Medicaid

Qualifications

• Must meet all applicable state and federal requirements for this position.
• Possess a Licensed Social Worker, Licensed Clinical Social Worker, or Licensed Nurse certification.
• Prefer social work experience in a health care setting working directly with individuals.

Responsibilities

• Conduct an initial social service assessment including MMSE, social history, intake for MDS, and admit note.
• Review nursing charts and complete MDS within facility timelines.
• Develop a written plan of care for each resident identifying problems, needs and goals.
• Ensure care plans are used in daily social services and reviewed with nursing staff.
• Evaluate social and family information and assist in determining plans for social treatment.
• Assist in developing, administering, and coordinating department policies; stay updated on regulations.
• Participate in discharge planning and development of social care plans.
• Provide information to residents/families regarding Medicare/Medicaid and other financial assistance.
