Job summary

Woodside in Molalla, OR is seeking a dedicated Social Services Assistant to support planning, development, and evaluation of facility social service programs. You will collaborate with administration, medical staff, and community agencies to meet residents’ medically related social and emotional needs.

You will assist in policy development, discharge planning, and coordinating resources while maintaining confidentiality and supporting residents’ independence and well-being.

Qualifications

• Must possess, as a minimum, a high school diploma or its equivalent; Associate or Bachelor's Degree preferred but not required.
• Prior health-care experience preferred.
• Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
• Ability to write reports, business correspondence, and procedure manuals.
• Ability to effectively present information and respond to questions from managers and employees.
• Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations.
• Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
• Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

Responsibilities

• Assist in planning, developing, organizing, implementing, evaluating, and directing social service programs.
• Meet with administration, medical and nursing staff in planning social services.
• Assist in developing and coordinating department policies and procedures.
• Review and update department policies at least annually.
• Coordinate social service activities with other departments.
• Assist residents in achieving the highest practicable level of self-care and well-being.
• Provide information to residents/families about Medicare/Medicaid and other financial assistance programs.
• Record and maintain Social Service progress notes and treatment responses.
• Assist in discharge planning with appropriate agencies and referrals.
• Develop and maintain good working rapport with other departments and external agencies.
