Qualifications

• Associate degree and 2+ years of experience preferred OR 4+ years of relevant experience.
• CNA or CMT certification preferred.
• CPR or BLS certification preferred.
• Experience in a similar role, preferably within a senior living community or healthcare setting preferred.
• Proficiency with Microsoft Office, EHR, and HR systems.

Responsibilities

• Develop resident-directed programming with guidance from the Director of Nursing and Executive Director.
• Assess resident needs at move-in and integrate goals into the service plan.
• Coordinate orientation and identify changes in the individualized service plan.
• Lead and mentor care staff and families on the care plans and dementia best practices.
• Coordinate schedules and ensure adequate care staff coverage.
• Organize staff orientation, mentorship, and ongoing training.
• Promote resident wellbeing and independence where possible.
