Job summary

Erickson Senior Living in Charlestown is seeking a Licensed Practical Nurse (LPN) to provide clinical care for residents according to nursing standards and Erickson Senior Living policies.

The role involves building relationships with residents and families, assessing health status, updating care plans, supervising care staff, administering medications, and participating in care meetings to ensure high-quality outcomes.

Qualifications

• Minimum 1 year of experience in assisted living, home health, or long-term care is preferred.
• Licensed by State as a Licensed Practical Nurse.
• CPR certification required.
• Experience with electronic medical records is preferred.

Responsibilities

• Develop relationships with residents and families to support holistic care.
• Assess and document residents' clinical condition and notify provider as needed.
• Update care plans to reflect changes in needs.
• Supervise care associates.
• Administer medications timely and accurately.
• Participate in meetings related to resident outcomes and care.
• Respond professionally to residents, families, or responsible parties.
• Review clinical indicators such as falls and wounds.
• Perform other duties as assigned by RN clinical leader.
