Job summary

Charlestown by Erickson Senior Living is seeking a Licensed Practical Nurse (LPN) to deliver compassionate clinical care to residents in our neighborhood. You will follow nursing standards, document assessments, and collaborate with the RN and physician to update care plans.

Qualifications

• 1 year of experience in assisted living, home health, or long-term care preferred.
• Licensed Practical Nurse licensed by State.
• CPR certification required.
• Passion for caring for seniors and strong resident advocacy.
• Experience with electronic medical records preferred.

Responsibilities

• Develop relationships with residents and families applying consistent caregiver approach.
• Assess, observe, and document residents' clinical condition; notify RN/provider when needed.
• Update or modify care plans reflecting changes in needs.
• Supervise and lead care associates.
• Administer medications timely and accurately.
• Participate in meetings related to resident outcomes and care.
• Respond to residents and families in a timely, professional manner.
• Support review of clinical indicators (falls, wounds, nutrition, etc.).
• Take on other duties as assigned by RN clinical leader.

Join our team as a Licensed Practical Nurse (LPN). The LPN is responsible for providing clinical care to residents in the neighborhood according to the nursing standards of practice, board of nursing, and Erickson Senior Living standards.

Compensation: Ranging from $32.00 per hour to 39.00 per hour; based upon years of licensure. Potential for $2.00 per hour shift differential for evening and night shifts.

How you will make an impact

• Developing relationships with the residents and their families using a consistent caregiver approach supporting the resident’s physical, spiritual, emotional, and clinical needs and focusing on placing the resident first.
• Assessing, observing, and evaluating resident’s clinical condition, document findings in the resident electronic medical record, and notify the provider and/or RN when necessary.
• Updating or modifying care plans as needed to reflect any changes to a resident’s care and service needs.
• Supervising and leading care associates
• Administering medications timely, accurately, and according to orders.
• Participating in meetings related to resident outcomes and care
• Responding to residents, family, or responsible parties in a timely and professional manner
• Participating in the review of clinical indicator reviews – falls, wounds, nutrition, antipsychotic use, etc.
• Other duties as assigned by RN clinical leader

What you will need

• Minimum of 1 year of experience in assisted living, home health, or long-term care is preferred.
• Licensed by State as a Licensed Practical Nurse.
• CPR certification required.
• A passion to care for seniors and be a strong resident advocate.
• Experience with electronic medical records is preferred.
