Job summary

The Hermitage in Sylva, NC seeks a Resident Care Coordinator to support residents' health and well-being by coordinating care services, developing and adjusting care plans, and ensuring regulatory compliance. • Active LPN or RN license preferred (state dependent).
• Minimum 2 years in senior living or healthcare setting.
• Experience supervising staff is highly desirable.
• Strong leadership and communication skills.

Responsibilities

• Develop and manage individualized service plans with clinical staff and families.
• Supervise caregiving staff and support scheduling for quality care.
• Engage residents and families; coordinate care conferences.
• Ensure regulatory compliance and participate in quality assurance.
• Collaborate with nursing, physicians, and therapists to coordinate care.
