Job summary

Envision Unlimited is seeking a Direct Service Professional to provide direct service and coordinate activities for residents in a Community Integrated Living Arrangement (CILA) in Homewood, Illinois.

Responsibilities include training residents in daily living skills, supporting independence per the IHP, and coordinating schedules; must have DSP training and a valid driver's license. TABE testing is required within 120 days of hire.

Qualifications

• Must have proof of Direct Service Professional (DSP) training.
• Must pass TABE test within 120 days of hire.
• Must have a valid driver's license and ability to drive agency vehicles.

Responsibilities

• Evaluate and provide training to residents in daily living skills (hygiene, grooming, housekeeping, laundry, meals, budgeting, socialization, recreation).
• Assist residents in achieving maximum independent functioning per the Independent Habilitation Plan.
• Implement programs to promote growth toward independent living skills.
• Plan daily schedules including chores, social activities, and appointments.
• Complete and ensure timeliness of progress reports, MAR sheets, incident reports, etc.
