Job summary

Commonwealth Senior Living in Haddam, CT is seeking a Resident Care Director (RN) to lead the resident care team and coordinate care for residents. The role interfaces with hospitals, physicians, and community organizations to deliver optimal care and ensure regulatory compliance.

Qualifications

• Must hold an unencumbered state RN license.
• Minimum of two years’ college in human services or health care field.
• Minimum of one year nursing management experience, including hiring, coaching, performance management, scheduling, and daily resident care operations.
• Experience with resident assessment and care planning.
• CPR and First Aid certification.
• Must be willing to become trained in state assessments and service plans as per regulatory licensing requirements.
• Knowledge of infection control practices and prevention of disease transmission.
• Ability to work weekends, evenings, and flexible hours as needed for resident care and services, including on-call responsibilities.
• Must be proficient in basic office equipment and driving.

Responsibilities

• Model a business philosophy that treats every resident and family member as a personal customer.
• Promote health and wellness of residents by partnering with physicians and care providers for integrated care.
• Ensure provision of services in accordance with federal and state policies.
• Collaborate with administration on referral development and occupancy goals.
• Establish rapport with residents, families, and staff across departments.
• Conduct routine resident care rounds to ensure needs are met per care plan.
• Complete assessments for residents at least biannually or with condition changes.
• Monitor and report resident care quality data to identify risks.
• Oversee community medication management and staff training.
