Job summary

Pikes Peak Post Acute in Oregon seeks a dedicated care coordinator/nurse to communicate Care Plans with residents and families. You will attend morning meetings, answer questions about care, monitor statuses, and organize daily work to meet changing needs.

This role requires completing documentation, performing utilization reviews, coordinating admissions, discharges, and referrals, and maintaining professional interactions with residents and staff.

Qualifications

  • Bachelor’s Degree in Nursing or Social Work required.
  • RN license preferred; LVN/LPN license acceptable.
  • Two years of clinical nursing experience preferred.
  • Knowledge of Medicare, Medi cal and Medicaid programs and benefits.
  • Ability to read and interpret policy and procedure manuals.
  • Excellent communication with residents, families, staff and partners.

Responsibilities

  • Communicate plans of care with residents and families.
  • Attend morning meetings to coordinate care with the team.
  • Monitor and adjust resident statuses based on changing needs.
  • Complete required documentation and utilization review activities.
  • Coordinate admissions, discharge, and referrals.
  • Ensure timely charting and paperwork.
