Job summary

Pikes Peak Post Acute seeks a qualified nurse/care manager to lead clinical assessments, care planning, and direct coordination of medical services. You will collaborate with residents, families, physicians, payers and providers to deliver extraordinary care while managing costs.

Responsibilities include attending daily stand-ups, communicating plans of care, ensuring timely documentation, coordinating admissions/discharges, and performing utilization reviews in a high-volume setting.

Qualifications

• Bachelor's Degree in Nursing or Social Work required or equivalent.
• RN license preferred; LVN/LPN acceptable with relevant experience.
• Knowledge of Medicare/Medi-Cal/Medicaid beneficial.
• CCM certification considered an asset.

Responsibilities

• Communicate plans of care with residents and families and healthcare providers.
• Attend morning meetings to coordinate with the team.
• Answer questions about care, treatment plans, and illness progression.
• Monitor resident statuses and change plans as needed.
• Organize daily work and prioritize resident needs in the area of responsibility.
• Complete documentation as required and maintain chart accuracy.
• Perform utilization review to ensure cost-effective care.
• Coordinate care with residents, providers, and payers; oversee admissions and discharges.
• Coordinate referrals to and from the facility; work with physicians and staff.
