Job summary

Escondido Pa in Forest Grove, OR is hiring a Resident Care Manager to lead clinical planning and care coordination in a post-acute setting. The role requires collaboration with residents, families, and healthcare providers to deliver extraordinary, cost-conscious care.

The position emphasizes thorough documentation, utilization reviews, and seamless admissions/discharges while maintaining strong teamwork and professional standards.

Qualifications

• Bachelor’s degree in Nursing or Social Work required.
• RN license preferred; LVN/LPN acceptable.
• Minimum two years clinical nursing experience.
• Knowledge of Medicare/Medi-Cal/Medicaid programs and benefits.
• CCM certification a plus; continuing education/licensing kept up to date.

Responsibilities

• Communicate plans of care with residents and families, including PT/OT and treatment protocols.
• Attend morning meetings to facilitate team communication.
• Answer residents' questions about care, illness progression, and treatment plans.
• Monitor and adjust resident statuses based on changing needs.
• Prioritize daily work by assessing resident needs and discharges.
• Complete documentation and perform utilization reviews to ensure cost-effective care.
• Coordinate referrals with residents, care providers, finance, and third-party payers.
• Oversee admissions and discharge activities; ensure smooth transitions.
• Promote collaboration among residents, families, physicians, nurses, and staff.
• Maintain organized charting and timely paperwork.

Education

Bachelor's Degree in Nursing or Social Work

Registered Nurse (RN) license preferred

Licensed LVN or LPN acceptable

Two years clinical nursing experience

Knowledge of Medicare, Medi-Cal and Medicaid

CCM certification a plus
