Job summary

Avamere Living in Hillsboro, OR seeks a Resident Care Coordinator (RCC) to coordinate resident care and staffing, maintain schedules, and support the Director of Health Services. You will supervise care delivery in alignment with policies and regulatory requirements.

Responsibilities include staff scheduling, training Caregivers and Med Techs, maintaining medical records, and participating in quality assurance meetings.

Qualifications

• Must have active CPR/First Aid certification during employment.
• Experience as Caregiver or Medication Tech in memory care/assisted living.
• Ability to obtain a Food Handler's Card.
• Experience with electronic medical records documentation.

Responsibilities

• Document changes in residents' status, needs, and appointments.
• Ensure proper documentation of care plans and nurses' notes.
• Provide orientation and on-the-job training to Caregivers and Med Techs.
• Assist with daily living activities (bath, dress, eat).
• Conduct weekly, monthly, quarterly audits.
• Complete quarterly resident evaluations as assigned.
