Job summary

Avamere Living is seeking a Resident Care Coordinator (RCC) to coordinate resident care needs and staffing, maintaining schedules and clinical records. This role supports the Director of Health Services and ensures compliance with policies and regulations.

The RCC will train caregivers and med techs, manage on-call coverage, and participate in audits and quality meetings. Prior CNA, CPR/First Aid, and EMR experience are preferred for a compassionate, professional team player.

Qualifications

• High school diploma or equivalent required.
• Prefer CNA license and Memory Care/Unit experience.
• CPR/First Aid certification required during employment.
• Experience with electronic medical records/documentation.
• Strong communication with residents, families, and staff.

Responsibilities

• Document changes in residents' status, needs, and appointments.
• Ensure documentation of care plans and nurses' notes.
• Provide orientation and training to Caregivers and Med Techs.
• Assist with daily living activities (bathing, dressing, eating).
• Conduct weekly, monthly, and quarterly audits.
• Complete quarterly resident evaluations as directed.
• Support staffing schedules and on-call coverage.
• Participate in morning stand-up meetings with feedback.
• Create a warm, patient, calm, and joyful environment.
• Maintain a clean and organized facility.
