Job summary

Frontier Senior Living is seeking an outstanding Resident Care Coordinator to join the Grand Montecito Memory Care community in Las Vegas, NV. The role requires coordinating care plans, supervising caregiving staff, and supporting daily operations to ensure quality resident care.

You will work with the Executive Director and RN, participate in staff training, and ensure regulatory and documentation standards are met. CPR certification and strong English communication are preferred.

Qualifications

• Familiarity with elderly population
• Experience in senior care and supervising staff required
• High school graduate preferred
• CPR certification preferred or required

Responsibilities

• Coordinate care plan conferences with residents and families
• Maintain caregiver task lists and update as ADLs change
• Schedule service plan dates according to state regulations
• Coordinate home health, hospice and ancillary providers; document visits
• Complete MAR audits as directed by RN
• Monitor the Medication Room for organization and cleanliness
• Assist RN with incident reporting and investigations
• Support Executive Director and RN in staff hiring, training, management
• Fill in for Receptionist, Medication Aide, or Caregiver as needed
• Manage day-to-day operations as manager on duty
• Maintain monthly budget for health services department

The Resident Care Coordinator requires an understanding and commitment to the principals and values of the community as well as an ability and willingness to train caregivers in tasks required for delivering quality care to the residents of the community.Primary Duties and Responsibilities:Conducts Coordination of Care plan conferences with each Resident and Responsible family membersMaintains current tasks lists for caregivers, and updates timely as ADL changes occurSchedules the service plan dates according to state regulation guidelinesOrders supplies as well as durable medical equipment as neededAssure home health, hospice and ancillary care providers, communicate their visits and provide necessary documentation of their visit.Completes MAR audits as recommended by RN.Monitors the Medication Room to assure organization and cleanliness.Assists RN with tracking and recording incident reports.
