Job summary

Rockingham County is seeking a Resident Care Coordinator to provide personal care and medication administration for residents at Ernest P. Barka Assisted Living Community.

The role collaborates with an interdisciplinary team to deliver high-quality, person-centered care and to support residents’ wellness goals. Responsibilities include vital signs, admissions documentation, and executing individualized care plans while maintaining infection control standards.

Qualifications

• Licensed nurse in NH (LPN/RN) required.
• CPR/BLS certified.

Responsibilities

• Provide personal care treatments and administer medications according to care plans and physician orders.
• Take vital signs, assist with resident admissions and documentation, and initiate individual service plans.
• Observe and report changes in residents’ condition to the Director of Resident Care.
• Document narcotics counts, shift reports, and MAR accuracy; follow infection control practices.
• Communicate with residents, families, physicians, and health service agencies to support care.
