Job summary

Rockingham County, NH’s Resident Care Coordinators provide medication administration, treatments, and personal care as part of an interdisciplinary team at Ernest P. Barka Assisted Living Community.

The role requires a NH-licensed nurse (LPN/RN), CPR/BLS, and at least three years in health care with one year in supervision; the position emphasizes high-quality, person-centered care, documentation, and collaboration with residents, families, and care partners.

Qualifications

• Minimum of three (3) years full-time health care experience with at least one (1) year in a supervisory role.
• Equivalent combination of work and experience will be considered.
• Licensed nurse in the state of New Hampshire (LPN/RN) required.
• CPR / Basic Life Support (BLS) certified.

Responsibilities

• Provide medication administration and personal care following established care plans and physician orders.
• Observe and communicate significant changes in residents’ condition to the Director of Resident Care.
• Complete required documentation, narcotic counts, shift reports, and MAR accuracy checks with infection control practices.
• Participate in Quality Improvement and Risk Management programs and family care conferences.
• Support admissions, documentation, and maintain professional, respectful resident interactions.

SCOPE OF POSITION:

Resident Care Coordinators play an important role in ensuring the wellness needs of Ernest P. Barka Assisted Living Community residents are met. They provide personal care treatments and medication administration while working as part of an interdisciplinary team to deliver consistent, high-quality, person-centered care. By building authentic relationships with residents, families, and care partners, they help residents achieve their short- and long-term wellness goals.

We are committed to providing preventative, restorative, and person-centered care to all residents at Rockingham County. We want our residents to feel safe, comfortable, and supported in all aspects of their lives to include not only physical, but also emotional, social, spiritual, environmental, and intellectual well-being. Our Resident Care Coordinators are a vital part of our dynamic team! They provide medication administration, treatments, and personal care while following established care plans, physicians’ orders, and community policies and procedures. They take vital signs, assist with resident admissions and documentation, and initiate individualized personal service plans to proactively manage each resident’s care and services. Resident Care Coordinators communicate with residents, families, physicians, and health service agencies to support positive communication and relationships. They are responsible for observing and communicating significant changes in a resident’s condition, environment, or other concerns to the Director of Resident Care. They complete required documentation, narcotic counts, shift reports, and monthly MAR accuracy checks while following infection control practices. Resident Care Coordinators also help ensure residents are treated with respect and dignity as their healthcare needs change. They participate in Quality Improvement and Risk Management programs, family care conferences, community events, and wellness activities. By encouraging teamwork and supporting their coworkers, they contribute to a caring, positive environment focused on providing high-quality service to residents.

Required Education And Experience

• Minimum of three (3) years full-time or equivalent experience in a Health Care setting with a minimum of one (1) year in a supervisory role; or
• An equivalent combination of work and experience will be considered.
• Licensed nurse in the state of New Hampshire (LPN/RN) required.
• CPR / Basic Life Support (BLS) certified.

Shift Differential

• 2nd Shift: $2.75/Hour
• 3rd Shift: $3.50/Hour
• Weekend Premium: $2.00/Hour
