Qualifications

• Unencumbered LPN or RN license is required.
• Previous long-term care experience is preferred.

Responsibilities

• Provide daily care to residents.
• Assist with dining needs and respond to call lights promptly.
• Chart and document resident care activities.

• Lead and coordinate resident care with a rehabilitation focus.
• Provide leadership for staff while supporting high-acuity residents.
• Ensure quality management practices are followed.
• Perform resident assessments and maintain documentation.
