Responsibilities

• Directly assist residents with daily tasks and monitor care changes.
• Aid residents with mobility using proper techniques and equipment.
• Perform routine resident care rounds per individualized care plans.
• Assist with lifts, moves, and transport with safe body mechanics.
• Maintain residents' skin integrity and change incontinent residents as needed.
• Document daily care activities per policy and standards.
• Report issues promptly to supervisor and respond to inquiries.
• Familiarize with individualized care plans and provide support accordingly.
• Contribute observations to care planning and resident preferences.
• Interact respectfully with residents, families, staff and supervisors.
