Job summary

Day Brook Nursing Center in Holyoke, MA seeks a Resident Assistant to support residents who require 24-hour oversight and assistance with daily living activities, while promoting independence and choice.

The position is non-exempt with an emphasis on compassionate, individualized care. The role involves following care plans, documenting care in health records, and collaborating with a nursing team to ensure safety, dignity, and well‑being for residents with dementia and other needs.

Qualifications

• High School Diploma or equivalent.
• Certified Nursing Assistant certification preferred but not required
• Completion and proof of a PCA/HHA training program that meets the 54 training hour requirement from EOEA
• Previous experience working in long term care/assisted living community preferred
• Previous experience working with dementia residents.

Responsibilities

• Orients and mentors newly hired resident assistants.
• Provides individualized attention to maintain residents’ physical, mental and psychosocial well‑being.
• Follows the individualized care plan and supports planning with nursing staff.
• Assist with activities of daily living and mobility.
• Maintain residents’ comfort, privacy and dignity and protect confidentiality.
• Prepare residents for meals and assist with feeding as needed.
• Record documentation in the resident health record and report changes to the nurse on duty.
• Ensure safe and clean environment, observe changes in condition and report promptly.
• Adhere to safety, infection control and care policy and participate in required training.
