Job summary

Sullivan Park Care Center is seeking a licensed CNA to provide routine daily nursing care in accordance with each resident’s assessment and care plan, under supervision. You will assist with daily living activities, monitor residents, and help maintain a caring environment that respects privacy and dignity.

Responsibilities include proper identification before care, accurate documentation, reporting changes to the Nurse Supervisor, and following facility policies.

Qualifications

• Must be a licensed Certified Nursing Assistant in accordance with state laws.
• CPR certification preferred but not required at hire.

Responsibilities

• Identify residents before treatments or meals; follow authorized abbreviations.
• Record entries accurately on flow sheets and charts.
• Report changes in resident condition promptly to nursing supervisors.
• Assist residents with daily living activities to maintain comfort and dignity.
• Maintain a clean, safe environment and protect resident confidentiality.
