Job summary

Normandy Nursing Center is seeking a Social Services Assistant to coordinate discharge planning within our hospital system and to support resident care and admission processes. You will develop measurable plans to enhance social services and work closely with residents, families, and other departments.

The role emphasizes teamwork, effective communication in English (bilingual preferred), and adherence to residents' rights.

Qualifications

• Bachelor's degree in social work preferred.
• Current state clinical social work licensure required.
• Two years of Social Services or behavioral setting experience preferred.
• Ability to work cooperatively as a member of a team.
• Able to communicate effectively in English, both verbally and in writing.
• Bilingual preferred.
• Basic computer skills / knowledge.

Responsibilities

• Coordinate social service/discharge planning within the hospital system by effective communication at all levels.
• Develop specific, measurable plans and objectives to enhance social service delivery.
• Perform psychosocial assessments and reassess residents.
• Assist with admissions and discharge planning processes to maximize resident satisfaction.
• Maintain relationships with referring community agencies and promote facility outreach.
