Job summary

Normandy Nursing Center is seeking a Social Services Supervisor to coordinate discharge planning, assess residents’ psychosocial needs, and collaborate with care teams to support safe, high‑quality resident care.

Qualifications

• Bachelor’s degree in social work or a bachelor’s degree in a human services field including but not limited to sociology, special education, rehabilitation counseling, and psychology
• Current State Clinical Social Work Licensure
• Ability to read, write and speak English
• Ability to work cooperatively as a team member
• Ability to communicate effectively with residents and families, and at all levels of the organization
• Ability to react decisively in emergency situations
• Maintain confidentiality
• Relationship management skills
• Knowledge of training techniques for clinical staff

Responsibilities

• Integrate social service/discharge planning within the hospital system through effective communication
• Develop specific, measurable plans and objectives to enhance social service daily
• Perform psychosocial assessment and reassessment for residents and families
• Support safe resident care and minimize health care errors
• Coordinate and direct Social Service/Discharge Planning Department to meet resident needs
• Provide resources and support to Social Service/Discharge Planning Staff/Clinical Staff
